The following documents on our checklist need to be completed and emailed to captive@delaware.gov

• Form A-1 Captive Application for Certificate of Authority
• Form A-2 Series Application
• Form B-1 Biographical Affidavit – needed for all Directors and Officers
• Form B-2 Declaration Affidavit for Financial Institution As Trust Trustee
• Form D-1 Designation for Receipt of Delaware Documents
• Form D-2 Designation for Service of Process
• Strategic Business Plan Outline
• Articles of Incorporation
• Bylaws
• Organizational Chart
• Minimum Capital and Surplus Guidelines
• Pro Forma Financial Projections (five-year expected and five-year adverse)
• Actuarial Study
• Financial Statements of Parent Company/Owners
• Policies, business agreements, insurance/reinsurance contracts and other vendor contracts as required for type of captive
• Additional attachments necessary to clarify and/or support statements or projections detailed in the Strategic Business Plan
• Supplemental license application materials for Branch, Sponsored and Series Captives
